Is this the exact real-time count?+
It is as current as YouTube's own public data. YouTube updates public view counts periodically and validates views before publishing them, so the figure tracks YouTube rather than a separate live stream of its own.

Does it work for live streams and premieres?+
Yes. It tracks the public view figures YouTube exposes for the video, including during a premiere and after a stream ends.
Why did the count pause?+
Click Stop to pause polling and Restart to resume. Counts can also plateau on their own while YouTube validates a burst of views — that is YouTube's behaviour, not the counter stalling.
